Sinossi
Dopo aver fatto uccidere suo padre e la sua perversa matrigna dai cenobiti, Kirsty Cotton viene mandata in un ospedale psichiatrico. Anche dopo aver cercato di convincere le autorità di ciò che è realmente accaduto alla sua famiglia, nessuno crede alla sua versione della storia e decidono di metterla nell'istituto, affinché si riposi e si rilassi un po'.
L'ospedale è comandato da un brillante e strano psichiatra, il dottor Channard, che da tempo cerca la chiave per un'altra dimensione. L'unica persona che crede in Kirsty è un giovane e gentile assistente del dottor Channard, chiamato Kyle MacRae. Seguendo la versione della storia di Kirsty, Channard mette le mani sul materasso insanguinato dove è morta la sua matrigna Júlia, Channard decide di resuscitarla, uccidendo i suoi pazienti e offrendoli come cibo a Júlia.
Lei ritorna senza pelle, e decide di aiutare Channard a portare le forze del male, ma i piani dei due saranno disturbati da Kirsty, che vuole porre fine una volta per tutte ai cenobiti, e da Kyle, che non vuole vedere Kirsty ferita...
