Unsane follows the story of Sawyer Valentini, a troubled woman who moves away from home to escape a stalker. Despite her efforts to start fresh, Sawyer finds herself still triggered by interactions with men due to her past experiences.
Seeking help, she makes an appointment with a counselor at Highland Creek Behavioral Center. However, during her appointment, she unknowingly signs a release that voluntarily commits her to a 24-hour stay at the facility.
When Sawyer tries to call the police for help, they refuse to intervene after seeing the signed release. After getting into physical altercations with both a patient and a staff member, Dr. Hawthorne informs her that she will be kept for an additional seven days.
As Sawyer navigates her time at Highland Creek, she learns that the facility is actually running a scheme to profit from health insurance claims. Patients are tricked into voluntarily committing themselves, with the promise of being released once their insurance runs out.
Things take a dark turn when Sawyer recognizes her stalker, David Strine, working at the facility under the alias George Shaw. This discovery leads to Sawyer being restrained and sedated due to her outbursts.
Throughout the film, Sawyer's struggles with the system at Highland Creek escalate, leading to repeated use of restraints and sedation as she fights to prove her sanity and escape the dangerous situation she finds herself in.
